Appendicitis doesn’t always cause the classic pain in the lower-right side of your abdomen. Most people associate appendicitis with sharp pain in the lower-right abdomen, but the appendix isn’t positioned exactly the same way in everyone. It can sit behind the colon, extend toward the pelvis, or occupy another slightly different position. Because of these anatomical differences, inflammation may sometimes produce pain in the back, pelvis, flank, or another unexpected area. Pregnancy can also shift the appendix from its usual location. This is one reason doctors consider several symptoms, physical examination findings, blood tests, and sometimes imaging rather than relying solely on where a person feels pain.
