A blood disorder can make urine dramatically darker after sleep. Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired blood disorder in which blood cells lack certain protective surface proteins. Red blood cells can consequently be destroyed by the immune system’s complement system. In some patients, hemoglobin released from destroyed cells can produce noticeably dark urine, historically associated with the morning hours.
